Small employers with fewer than 50 full-time employees who offer self-insured health coverage must file 1095-B. This includes self-insured plans, level-funded plans, and certain HRAs. Employers with fully-insured plans do not file 1095-B (the insurer files instead).

Form 1095-B reports minimum essential coverage and is filed by insurers and small self-insured employers. Form 1095-C is filed by Applicable Large Employers (50+ employees) and includes offer of coverage information. Small employers file 1095-B, not 1095-C.
